REQUESTING MEDICAL RECORDS

 

 

If you require medical records, please fill out the Medical Records Request Form. You will need to fax, mail, or drop off the completed form to one of our offices. Our Medical Records Coordinator will be in touch with you shortly after receiving it. Please note that while we make every effort to process and complete your request as quickly as possible, some requests may take up to 14 business days to complete.

ATTORNEY RECORDS

 

If you are an attorney and need to request medical records on behalf of a client, please fill out the Texas Attorney General Form below and send to MRR@ctpomd.com.

AMENDMENTS

 

We respect the right of our patients to request an amendment or correction to your medical records. Please use the form below to make a request to our practice. We will consider all requests made and will respond to all requests.
